Other clinical and administrative team members include:
- Dedicated ambulatory and inpatient Social Workers who work with each patient to enhance quality of life and outcomes.
- Transplant Dieticians who provide transplant-specific education and nutritional support to patients and their families pre- and post-transplant.
- Financial Coordinators who are available to help guide our patients through the insurance process and provide education on the financial aspects of transplant – from evaluation to life after transplant.
- Outreach Coordinators who assist with transplant education for patients, provide outreach and support for community health care providers, and facilitate ongoing communications between Hume-Lee and referring providers.
